Empowered Lives Program

Tenancy Support

 Support people with a disability in their goals to source, secure and maintain appropriate accommodation. Though this, we build the capacity of the family and participants to support a person with a disability to increase their independence through supporting their transition to live independently, accessing affordable furniture and home necessities and maintaining their tenancy with ongoing support from our excellent team.

Initial Supports

  • Assessment of tenancy requirements 
  • Initial screening for suitability for private rental or social/public housing
  • Assistance with completing applications, other paperwork and budgeting

Supports on successful private rental application

  • Assistance with obtaining Bond loan and organising move
  • Support to set up Centrepay for rent and utilities

Support for Social/Public Housing applications

  • Support with documentation, application forms, letters of support/references, etc.

Continuing tenancy support options as per NDIS funding

  • Support to maintain tenancy in accordance with Residential Tenancy Act 1997 (Residential Tenancies Amendment Act 2018)
  • Domestic services access (e.g. cleaning and gardening)
  • Communication/liaison with property managers/landlords
  • Support with rental inspections

Support Work

 Our Disability support workers provide care, supervision and support for people with disabilities in the home, residential establishments, clinics, hospitals and the community. They also work with other health professionals to maximise the individual's physical and mental wellbeing. They also provide companionship and emotional support, and when possible, promote independence and community participation. 

Skill Development

Connect You 2's Empowered Lives Program offers a variety of high quality Staff to who help to develop and build on interests, abilities and support community participation for participants.

The objectives of CONNECT You 2 skill development are :

  • Support participants to set and achieve individual goals
  • Support young people with transition into adult life
  • Build on current skills and assist participants to learn new skills
  • Encourage participants to maintain and develop peer relationships
  • Provide participants with choices and access to a wide range of activities and life experiences

What do we do?

At Connect You 2, we aim to provide a high standard of cleaning services to ensure a clean, safe and healthy house, and to ensure the outside of your home is tidy, kept and how you like it done. 

We support you with cleaning where you want us to, washing dishes, support with your laundry and many more! Our gardeners take care of your lawn, maintain bushes, support you in looking after your plants and remove weeds among many more tasks they can do.

Our Music Group

Wait a music group?

You heard that right - we have a music group! 

Every week we support participants who want to try to play an instrument or have passion in playing music to our weekly music group - our quality musical staff support and help you learn songs to play and as a group we all song together! We provide instruments, or you can bring your own if you like.

A lunch for the participants to all eat together is organised, cementing the inclusive nature of our music group.

Do you offer 1 on 1 support?

Yes we do! Receive 1:1 support from a professional musician/support worker - learn to play an instrument from scratch or enhance existing skills, learn more about the industry, attend

recording studio sessions, learn to maintain your instrument and more!
